potential alzheimer's patient. Since the idea of placement has started, what are the steps to take? Call a visiting nurse to evaluate patient? When talking to perspective homes, what else should the family bring with them? What are the financial rules when one spouse is staying in home, the other may be going to a home. What kind of coverage is there for having care given at home instead of a nursing home? thanks
I know you asked about information specific to New York. I don't have information on New York but I can tell you that unless you have long term care insurance or the person is going to the nursing home to be rehabilitated short term and will return to home you will have to pay for care out of pocket. Medicare will pay for rehabilitation care for 90 days unless the facility feels the patient is no longer making progress. I live in Kansas and my Mom (who just passed away from Alz.) had to pay over $100K for one year of care in a nursing home. Here in KS all of the patients assets must be liquidated before they are eligbile to go on Medicaid. In my experience having a home health aid come to the home is far more expensive then a nursing home. It depends on the level of care needed. Also in my experience home nursing care is 100% out of pocket cost.
The patients doctor will have to evaluate them and recommend that they go to a nursing home. I would start touring nursing homes ASAP. You can look up nursing home ratings if you look up New York Nursing home ratings online. Don't make appointments to tour them, just show up unannounced. Any reputable home will not have a problem with this. Also nursing homes have social workers that can help you with all of your questions that are in regards to you specific state.
If you loved one is currently living at home I am assuming they are not 100% dependant on someone....you might want to consider a less expensive option of Assisted Living. Here in KS assisted living is about $4000 less a month then a nursing home. Most will provide 3 meals, a locked unit so Alz. patients can't get out an get lost. They also provide help with medication, toileting needs, dressing/grooming, just to name a few services they offer. Generally a person can stay at an assited living as long as they don't require more then one person to move them from a bed to a wheelchair or what is called a more then a 1 person transfer.
One more thing....when you go to look at nursing homes they will tell you the daily bed charge, but when you actually get the bill it will be a lot more. Don't be shocked. The actual cost will include medications, medical supplies etc which will really jack up the monthly bill. It is really ashame that nursing homes are allowed to rip sick elderly people and their families off.

My sister feels that I can ride with my mother in a cab to the nursing home alone without a nurse. My mother is unable to walk on her own and suffers from Parkinsons disease and Homeostatic hypertension. I told my sister that I feel that a nurse should ride with my mother to the nursing home to make sure she arrives safely. What do you think?
She is flying on the plane with a nurse, she cannot fly alone.
Your mother us going to have a difficult time at the airport even with a nurse. She can become more stressed by the chaos around and therefore more symptomatic by the time she lands. Then there is the wait for luggage and a taxi. Please understand that if your mother is going to a nursing home she is at a stage where she cannot live alone and certainly should not travel alone. I assume that since the nurse is flying with her, she will be much more comfortable with someone helping her through the process but it is still exhausting.
Will she be in a wheelchair? My experience with a wheelchair (I have PD) at the airport has been both positive and negative. One experience left me wishing I could stand throughout the process because I was searched thoroughly and suspect because I was in the wheelchair. Your grandmother should have a wheelchair, however, because she has Orthostatic Hypotension, often a side effect of PD medications, which could cause her to become dizzy and possibly fall.
You are absolutely correct that she should have a nurse accompany with her. Explain to your sister that Orthostatic Hypotension can also cause your mother to pass out. In a taxi? Alone? I think not.
The very fact that her destination is a nursing home should be sufficient. Your sister needs to understand that PD has changed your mother's reaction time, possibly her urinary processes, her ability to handle stress, even manage getting in and out of a cab. Travel basics are difficult enough without Parkinson's disease. She needs someone with her every step of the way. Would you trust your mother's medical care to a NY cabbie? (nothing against them but...)
Thank you so much for understanding. Stick to your guns. This is a right/wrong situation and your sister is remembering a mother from the past - perhaps she is in denial rather than simply not understanding. Perhaps she thinks your mom's blood pressure will be higher from the tension of the trip. But your sister needs to demonstrate, if not comprehension and compassion, than at least tolerance and let you make the arrangements.
I am trying to find a way to contact DON's at Nursing Homes in order to set up clinical trials for a new medical device. What is the best way to go about doing so (i.e. getting in the door.) Any advice/comments would be greatly appreciated.
